Registration Forms
Once your child is accepted into our program, you will need to complete a registration form and include a $100.00 (non-refundable) registration fee. This is usually done in person at the preschool office, as it allows you to see our program and ask questions.
Enrollment Forms
Once your child has been accepted into our program, and you have submitted your completed Registration Form, you will need to complete various Enrollment Forms. These forms (with the exception of the Medical Form) should be completed and returned to the Preschool Office by May 1. The Medical Form needs to be completed and signed by your doctor.This form is needed prior to the first day of school. If you have misplaced any of these forms, please download them at the link in the top right column of this page, complete them and return them to us via mail.
